Sign the Jump and Play waiver

In consideration of being permitted to enter the premises of Jump and Play and to use its trampolines, attractions, and equipment, I, the undersigned parent or legal guardian, on behalf of myself and each child listed above (collectively, the "Participants"), hereby acknowledge and agree to the following.

Assumption of risk. I understand that jumping, climbing, and play on trampolines and similar attractions carries inherent risks of injury, including but not limited to sprains, fractures, and other bodily harm, and I voluntarily assume all such risks on behalf of the Participants.

Release of liability.I hereby release, waive, discharge, and covenant not to sue Jump and Play, its owners, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of ordinary negligence in connection with the Participants' use of the facility.

Medical treatment authorization. I authorize Jump and Play staff to secure and administer basic first aid and, if necessary, to seek emergency medical treatment for the Participants, at my expense, should I not be immediately reachable.

Photo release. I grant Jump and Play permission to photograph or record the Participants during their visit and to use such images for promotional or marketing purposes without further compensation.

Rules of the facility. I agree that the Participants will follow all posted rules and staff instructions at all times, and that Jump and Play may refuse entry or remove any guest who does not comply with safety rules.

This waiver is valid for one year from the date signed and applies to every visit by the Participants during that period.
